Skin and Scalp Problems: Looking Beyond the Skin

Skin and scalp problems are often treated as problems of the skin alone. But sometimes, what we see on the outside can be connected to what is happening inside the body.

In this clinical consultation, I discuss a 22-year-old patient who was experiencing severe scalp scaling, sebopsoriasis/dandruff, hair thinning, facial flushing and back acne.

The important part of this case was not simply the skin symptoms. The discussion focused on looking for the underlying metabolic factors associated with these problems.

When Skin Problems May Have an Internal Connection

The patient had significant scalp scaling along with other skin concerns. In the consultation, I explain the connection between these visible symptoms and excess visceral fat, fatty liver and insulin resistance.

The focus was therefore not just on treating the skin from the outside.

When there is an underlying metabolic problem, addressing that problem becomes an important part of the overall approach.

Insulin Resistance and Skin Health

Insulin resistance was an important part of the discussion in this case.

The consultation highlights how metabolic health can be connected with inflammatory changes in the body and how these may be reflected through different skin and scalp problems.

This is why I believe it is important to look beyond the visible symptom.

A person may come with acne, dandruff, scalp scaling or hair thinning, but the complete picture may require looking at weight, visceral fat, fatty liver and insulin resistance as well.

How Dandruff Can Contribute to Back Acne

There is another practical connection discussed in this consultation.

When there is severe scalp scaling or dandruff, the flakes can fall onto the back. These flakes can contribute to clogged pores and back acne.

Therefore, in someone experiencing both significant dandruff and back acne, the scalp and the back should not necessarily be considered two completely unrelated problems.

Hair Thinning and Facial Flushing

The patient in this consultation was also experiencing hair thinning and facial flushing.

These symptoms were considered along with the scalp condition, back acne and the patient's overall metabolic picture rather than being viewed individually.

This is the approach I wanted to highlight through this case: look at the complete picture rather than concentrating only on the most visible symptom.

Diet Matters

The dietary discussion in this consultation was very clear.

I advised reducing and eliminating foods that can contribute to an unhealthy dietary pattern, particularly:

  • Maida or refined flour
  • Excess sugar
  • Processed foods

The objective was to address the underlying metabolic concerns rather than simply concentrating on the appearance of the skin.

Don't Treat the Skin in Isolation

Acne, dandruff, scalp scaling and hair thinning can be frustrating, particularly when they continue to return.

But this consultation demonstrates why it can be useful to ask a broader question:

What is happening inside the body that may be contributing to what we are seeing on the skin?

In this particular case, the discussion centred on visceral fat, fatty liver, insulin resistance, inflammation, diet and lifestyle.

The skin is visible. The underlying metabolic picture may not be.

That is why, when dealing with persistent skin and scalp problems, looking beyond the skin can be an important part of understanding the individual case.
